so this happened yesterday.

scared the poo out of me. tire came off, went across 4 lanes of traffic, about 75 yards away.

lucky...it didnt do more damage to the car, lucky the tire didnt hit anyone coming opposite direction, lucky i was close to home.

I changed lugs nuts(Dorman) and added center caps(speedway) about 1 week ago.

my best guess is that the center cap mounting flange wasn't allowing the lug nuts a positive friction to the beveled hole

they do, or did. They backed off. I called Speedway and asked them if they had heard of anything like this, of course they said no. But honestly I think its a design flaw in that mounting plate. It wasnt allowing proper lug nut to tapered wheel friction.

I'd bet a hundred bucks you are right.
You also got lucky the tire did not take out your rear quarter panel. I've seen that happen too many times.

THAT IS INSANE! Like who hasn't had this nightmare cross their mind, but when it does, oh unholy hell! Yes, so glad your quarter panel isn't blown apart.

Wow... glad no one was hurt. Happened to me after my dad put new tires on his 1968 Delta 88. Took him to work the next morning and on the way home the right front wheel fell off. Happened to Ron White too... NSFW

it did some damage, unfortunately its on the crease, this will be a little harder to fix.
